Practice Manager. A small fee will
be charged to cover administration
costs.
Patients are responsible for turning
up on time for their appointments,

to keep waiting times as prompt as
possible but if you are waiting for
more than 20 minutes do please tell
reception.
Patients will be

a two-way code of conduct. At no

adhered to.
All staff adhere
to strict rules and regulations
regarding confidentiality of

Any information held at the surgery
about you is NEVER sent to anyone
else without your prior signed

Companies etc).
Like all other public authorities we

of Information Act 2000, should you
have any queries regarding this

the Information Commissioner.
We are grateful to
the many patients who have
contributed to this fund. The
practice uses this fund to purchase
and maintain items of additional
medical equipment for use on our
patients in Coleford and the
surrounding villages. We have
equipment such as spirometer
machines (to help asthma sufferers),
nebulisers, a state-of-the-art
defibrillator, INR (warfarin doses),
a tympanometer (help diagnose ear
problems), 24hour blood pressure
machines etc. All of this equipment
helps reduce the need to attend
local hospitals as well as giving us
information much more quickly to
help diagnosis.
This equipment would not otherwise
be available without the generosity
of our patients and their relatives.
Our patients' library is situated in
the waiting room is also funded by
the amenity fund and has a varied
and interesting selection of books
and videos for patients to borrow
free of charge – feel free to have a
look. To borrow a book or video just
take it to reception and leave your
name and contact number. |
