WELCOME TO THE PRACTICE

Our partnership prides itself in providing a traditional, friendly, but efficient family doctor service from our modern computerised surgery in Coleford. Our small team of doctors, nurses, health visitor, midwife and administrative staff work closely together; not only to help when you are ill, but also to try to prevent serious diseases from occurring.

We will accept patients living anywhere in our practice area, as shown on the map held in reception, but essentially between the towns of Frome, Midsomer Norton, Radstock and Shepton Mallet.

We are a GMS practice (General Medical Services) and our PCT is
Somerset Primary Care Trust based in Yeovil.

HOW TO REGISTER AS A PATIENT

To register with the practice please ask for forms from reception. You will be registered (as per GMS2 contract April 2004) with the practice, not a specific GP. You can request your preferred GP for consultations on a regular basis, however please understand that sometimes specific GPs may have a slightly longer wait due to holidays/training etc.

Once an appointment is made you are responsible for turning up promptly, if you cannot attend please ring Reception to cancel – it can then be offered to someone else, even at short notice.
 
DISABLED ACCESS

Our surgery has been designed to provide easy access for the disabled and the elderly. A wheelchair is available at reception. We also have a wheelchair available for short-term loans for patient use outside of the surgery (just ask at reception). Disabled ladies can be offered well woman care and smears at home if they are housebound.  Hearing loops are on reception and a portable hearing loop can be taken into the consulting rooms.
