Our partnership prides itself in providing a traditional, friendly, but efficient family doctor service from our modern computerised surgery in Coleford. Our team of doctors, nurses, health visitor, and administrative staff work closely together; not only to help when you are ill, but also to try to prevent serious diseases from occurring.

We will accept patients living anywhere in our practice area, as shown on the map held in reception, but essentially between the towns of Frome, Midsomer Norton, Radstock and Shepton Mallet.

We are a GMS practice (General Medical Services) and our Clinical Commissioning Group (CCG) is Somerset CCG based in Yeovil.

How to register as a patient

To register with the practice please ask for forms from reception. You will be registered (as per GMS2 contract April 2004) with the practice, you will also be allocated a specific doctor who will be your named/usual GP.

Your usual GP is the clinician at the surgery who will have overall responsibility for your care. Patients who are over 75 years of age will have been informed already of their named GP and if you are newly registered you will be informed within 21 days of your registration.

If you express a preference as to which GP you wish to be assigned we will make reasonable efforts to accommodate that request. This may not always be possible as some of our doctors work part time and therefore they may have slightly longer waiting times. Please note although you are allocated a GP this does not prevent you from seeing any of the GP’s working in the practice.

Once an appointment is made you are responsible for turning up promptly, if you cannot attend please ring Reception to cancel – it can then be offered to someone else, even at short notice.

Out of Area Patient – you may still register as a patient with us even if you live outside of the practice area (catchment area). However there is no obligation for the practice to provide home visits and this will be explained at the time of registering.
